When at the appointment i said im there for meds managment.

Then i went to the Hawai'i State Library printed out Cage-aid form and i walked in there, and the Medical Assistant signed it last time i just did a walk in.

the Housing Questionare is for the Mental Health Case Manager to fill. i signed the four consents to (Mental health Kokua, Stead Fast and the others..) on the computer screen i met my mhc at Starbucks Alamoana mall past the USPS.
